Immunisation Performance & Outreach Coordinator

About the Role Are you passionate about improving health outcomes for our tamariki? Ngāti Porou Oranga (NPO) is seeking a dedicated Immunisation Performance & Outreach Coordinator to join our Primary Health Services team.

This is a critical, fixed-term role located in Tairāwhiti / East Coast region, focused on increasing immunisation coverage and ensuring no tamaiti is left behind.

You will provide targeted coordination of immunisation performance by identifying overdue tamariki, managing recall processes, and supporting our clinic teams to bridge immunisation gaps. This role is about more than just data; it is about proactive outreach and engagement with whānau to reduce inequities and improve access to care.

 

Key Responsibilities

  • Performance Monitoring: Regularly review immunisation data to identify trends,gaps, and priority areas for follow-up.
  • Recall Coordination: Lead systematic recall processes and ensure outstanding immunisation opportunities are actioned by clinic teams.
  • Outreach Support: Work collaboratively with community teams to identify and support whānau who may face barriers to accessing routine services.
  • Reporting: Deliver accurate weekly, fortnightly, and monthly reports on performance and recall activity to drive clinical action.
  • Continuous  Improvement: Identify barriers to immunisation and support clinics in developing practical, whānau-centred responses.

 

What We Are Looking For

  • Experience: A background in health, primary care, or a community/public health environment is essential.
  • Analytical Skills: The ability to interpret data, identify performance gaps, and translate that information into practical improvements.
  • Communication: Strong relationship-building skills with the ability to influence multidisciplinary teams.
  • Cultural  Competence: A deep respect for Ngāti Porou identity, tikanga, and the importance of kaupapa Māori health delivery.
  • Technical  Savvy: Proficiency with databases, spreadsheets, and reporting systems.
